Insurance Information

We will gladly file your insurance claim after each visit.Insurance can be a very confusing issue. As an added service to our patients, we will gladly file your insurance claim after each visit.

We are preferred providers with the following insurance companies:

United Concordia, Met Life, Guardian, Cigna, Delta Premier, Delta Preferred & Blue Cross Blue Shield of Kansas

By being a contracted provider, it will allow you to utilize the most benefits from your dental insurance carrier.

Don't see your insurance carrier's name on the above list?

Don't worry! While we may not have a preferred contract with all insurance companies, we will still be happy to file an insurance claim on your behalf.
To take the guess work out before your first visit, we can also obtain your eligibility and benefits prior to your first visit if you are able to provide us with with a few key pieces of information; subscriber name, date of birth, social security number or subscriber identification number, patient's name, date of birth and the insurance carrier's contact telephone number.

Please be assured that we will make every possible attempt to obtain the most accurate information possible from your insurance provider. After your first visit when you have received a diagnosis and treatment plan, we can file what is called a "predetermination of benefits". This predetermination will allow us to tell your insurance carrier what the desired course of treatment is along with our fees, and then in return, they will evaluate and reply as to the amount of your treatment that they will cover. This will help to eliminate any surprises during the course of your treatment.

Please be assured that we will make every possible attempt to obtain the most accurate information possible from your insurance provider. Please be advised that any information we are able to obtain is only an estimate from your insurance company and final determination of actual benefits will only be made after treatment is completed and a claim is submitted to your insurance carrier. Ultimately insurance is a contract between you and your insurance carrier, and you are responsible for any amount they do not cover.
